Coordinated Family Support Services are designed for adults (18+) eligible for Regional Center services, who are living with their family and would like support to coordinate services.
Eligible clients and/or their family must be willing to participate in at least one visit with Teaming Up per week (minimum 1-2 hours face to face time per week).
Every family has different needs. A unique service plan is developed through the referral process. Some examples of help Teaming Up can provide include:
Education about a client’s diagnosis and options for supports
Training to navigate resources and coordinate consistent care, including identifying backup providers
Creating plans to increase safety and independence
